Short Term Rehabilitation

After a person has a surgery, a fall, or maybe just an overall decline in health they need a little boost to get them back to their prior level of functioning. Here at St. Christina, our board certified Physical, Occupational, and Speech Therapists, along with our nursing team work together to create a customized rehabilitation plan specific to the person’s needs while communicating frequently with family along the way to discuss progress and to promote a positive experience.  Our goal with Short Term Rehabilitation is to get you or your loved one back to living life to the fullest.

St. Christina Nursing & Rehabilitation Community is a licensed Medicare provider. To qualify for Medicare Skilled Nursing benefit the following must be met:
  • Individual was admitted to an acute care hospital for at least 3 midnights
  • Must be admitted within 30 days of the discharge from the hospital or other Skilled Nursing Facility
  • The care and condition must require inpatient services, such as nursing, rehabilitation or both for treatment related to the original hospitalized condition

Long Term Care

St. Christina’s Interdisciplinary Team will assess and determine the care needs in conjunction with physician guidance. The nursing team determines the appropriate care level by assessing the amount of services your loved one needs, during the first few days after admission. This level of care is evaluated quarterly and upon a significant change in condition thereafter. We have a multi‚Äźdiscipline approach so we can customize the care being delivered to our residents’.  We do not believe in a blanket approach to one’s care, each person we take care of is unique and different and you care plan should reflect the same.
